DISCLOSURE - CAMPUS SAFETY AND SECURITY REPORT 

I am applying for admission as a student to Academy of Allied health, AAH (“Academy”), and have been informed that the Academy's Annual Campus Safety and Security Report (“Report”), published by October 1 of each year, is available on the Academy's website: https:// academyofalliedhealth.com The Report also contains the Academy's Policy on Alcohol and Drugs. To maintain compliance with Jeanne Clery Disclosure of Campus Security Policy and Crime Statistics Act (“Clery Act”), Higher Education Act (“HEA”) requirements, and related statutes and regulations, and in an effort to continuously promote and improve safety and security measures on campus, the Academy's Report contains the past three calendar years of crime statistics for the Academy's campus. The Report also details security policies, information on campus law enforcement, policies on the use, possession, and sale of alcoholic beverages and illegal drugs on campus, and potential crimes committed on campus. I have also been advised that a paper copy of the Report will be provided upon request by contacting the Campus Director. I have been informed that the Academy recognizes the danger sex offenders pose to student safety.
